Upon submission of the above data, I agree to avail of the the company's special discount privilege and as such agree to send the company copy of at least two (2) months of my Statement of Account (Phone Bills) from the providers of both my home and office wireless phone and fixed line services.

Upon submission of the above data, I agree to pay the company a "Good Faith Deposit" equivalent to 25% of my Total Communications Costs.

I understand that the above amount is only a Good Faith Deposit. It is fully refundable if the service is not consummated, and to be fully credited, however, to my first month billing costs.
